A savings plan, not insurance

“2 free cleanings” is defined as a routine adult prophylaxis procedure. This program is a dental savings plan, not dental insurance.
The initial medicated cleaning receives 20% off. Maintenance cleanings are included in the total yearly cost (4 per year). This plan is only covered at the office of Dental Concepts.
Payment is due when services are rendered.
This plan is non-refundable.
This membership cannot be used in conjunction with any other dental plan or offer. It is the sole responsibility of the member to maximize benefits by arranging the appropriate appointments within the 12 month membership period.
Membership is effective on the day on which payment is received. Membership rates may be reviewed and adjusted on an annual basis with notification.
If CareCredit is used to pay for services, the discount will be 10% instead of 20%.
The plan auto-renews on a 12 month cycle. The card on file is processed annually unless a written request to cancel is received 30 days prior to renewal.
