Position Overview: We are seeking a skilled and empathetic Medicare Admission/Visit Nurse to join our dynamic team. The ideal candidate will play a pivotal role in ensuring that our clients receive the best possible care from the moment they enter our program.

Key Responsibilities:

Conduct initial assessments and intake processes for new Medicare clients.
Develop and implement individualized care plans based on client needs.
Serve as a liaison between clients, caregivers, physicians, and other healthcare professionals.
Ensure compliance with Medicare guidelines and regulations.
Provide ongoing support and follow-up with clients and their families.
Maintain accurate and detailed documentation of all client interactions and care plans.

Qualifications:

Current and valid RN license in the state of Michigan.
Prior experience in home health care or a related field preferred.
Strong understanding of Medicare regulations and guidelines.
Excellent communication and interpersonal skills.
Compassionate, patient, and dedicated to providing exceptional care.
