Position: Coder II
Status: Per Diem / As Needed
Schedule: Variable days and flexible hours per the department needs.
Job Summary:
Responsible for reviewing, analyzing and accurately assigning ICD-10-CM codes as well as CPT codes for all inpatient and outpatient health records. Responsible for reviewing the medical record to assure that it has been appropriately and adequately analyzed and flagged for physician assistance in completion of the edical record.
Responsible for maintaining compliance with Memorial Hospital’s policies, procedures and standards of care. Responsible for maintaining compliance with Health Information Management specific policy and procedures.
Tasks include, but are not limited to:
1. Diagnosis coding for all services rendered, inpatient and outpatient
2. Procedural coding for outpatient and inpatient services when applicable
3. Auditing of charges for outpatient services
4. Assists in documentation audits as needed
5. Abstracting
6. Maintains current coding competence regarding ICD-10-CM, CPT and HCPCS coding guidelines, hospital guidelines/requirements, as well as third party payer guidelines and policies and CMS coding guidelines as applicable
7. Understands and utilizes physician query guidelines as necessary
8. Performance Improvement trending
9. Other duties as assigned by Director of Health Information Management
10. Trauma Registries (as assigned)
Qualifications:
Education:
RHIT accreditation, Certified Coding Specialist, Certified Professional Coder, or applicable college enrollment/completion of coding classes
Training:
1. Participate in continuing education programs as required by credentials
2. Ongoing training including but not limited to review of all policies and procedures pertinent to responsibilities
3. Mandatory training as necessary to meet the needs of the organization
Experience:
1. Experience in ICD-10-CM, ICD-10-PCS, CPT and HCPCS Level II.
2. Experience in coding of inpatient and outpatient encounters.
Job Knowledge:
Within 3 months of employment, employee must demonstrate knowledge, competency, and proficiency in performing the duties of this position as outlined in the job summary and tasks listed above. Must have comprehensive knowledge of medical diagnostic and procedural terminology. Must understand disease processes, anatomy and physiology necessary for assigning accurate numeric and alpha-numeric codes. Thereafter, employee must continue to maintain knowledge, competency, and proficiency in these skills.
Performance Requirements:
Physical Demands :
The position requires some prolonged sitting, some standing, bending, stooping, stretching and lifting (35# max). Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator, fax machine and other office equipment.
Responsibility for Errors, Losses and Process:
Any errors, losses, accidents, or injuries involving a patient, hospital staff, visitor, patient property, hospital property, or visitor property are to be reported to direct supervisor.
Organizational incident reporting procedures and procedures for disclosure to the patient and family will be followed.
Responsibility for Safety;
Employee must be knowledgeable and adhere to all safety policies. Employee will be responsible for environmental health and safety of self and others. Employee will be responsible for personal safety to prevent workplace accident and injury. Unsafe practices or acts observed are to be reported to direct supervisor.
Responsibility for Supervision of Others:
Does not have supervisory responsibilities.
